Family Medicine at South Waterfront is a community based clinic offering primary care and specialty services for all types of patients. We treat everyone--babies, children, adolescents, adults (including moms-to-be), and seniors. Our services include maternity care (including deliveries), women's reproductive health, behavioral health care and counseling, transgender care, pediatric care, travel immunizations, podiatry, sports medicine, acupuncture, minor procedures, and health maintenance with preventive care. Between 150 and 200 patients are seen per day by our 40 plus providers assisted by 60 plus staff members.
The Clinical RN at Family Medicine at South Waterfront is responsible for the delivery of appropriate, timely, and beneficial care for patients which promotes quality and cost-effective health care outcomes working within established standards. Provides standard nursing care for patients throughout the age continuum. Integrates factors related to quality, safety, efficiency, and cost effectiveness in planning, delivering, monitoring and evaluating client care promoting the most effective and efficient use of human and financial resources.
Clinical Skills and Duties:
Actively coordinates care for patient care as needed.
Reviews the visit schedule to anticipate healthcare needs of patient panel.
Responsible for handling urgent/emergent calls and walk-ins per clinic approved advice protocols.
Assists in managing nurse visit schedule which includes but is not limited to: B/P checks, wound care, suture removal, education/teaching, INR testing, prenatal intakes.
Clinical skills to include but are not limited to; INR testing, IV placement/infusion, bladder catheterization, suture removal, immunizations/injections, POCT.
Uses EMR to assist with patient care.
Cross coverage of all clinical teams as needed.
Follows standing orders/protocols in assisting with patient care.
Perform hospital follow up calls.
Expected Abilities of the RN:
Ability to perform duties using theory-based nursing practice model, evidenced-based practices, and shared governance in a multidisciplinary team model. Ability to participate in a multidisciplinary clinical team.
Knowledge of clinical quality improvement models and process.
Knowledge of scope of patient care services provided by other clinical service lines and the referral process.
Knowledge of effective teaching processes and principles in planning, providing and evaluating optimum health care.
Knowledge of quality management and improvement processes to comply with internal and external standards, i.e., DNV.
Knowledge of general medical procedures and the ability to anticipate complications and the indicated therapeutic interventions.
Knowledge of clinic practices and ability to act as a resource for clinic personnel and assist the providers as necessary.
Proficiency in the use of appropriate EMR, computer based software and office equipment.
Ability to maintain quality safety and infection control standards.
Knowledge and ability to apply primary care best practices.
Ability to mentor and orient new staff members.
Provides accurate and timely documentation of care of patients in the patient record - EMR.
Assure proper transfer of patient care and coverage of duties when leaving the clinic.
Initiate appropriate interactions with patients, providers and other staff regarding any relevant patient care or operational issues.
Policies and Procedures:
Implement agreed upon changes in function, policies and procedures.
Establish appropriate patient/clinic flow priorities and follow through to completion.
Participate in the formation, development, implementation, and evaluation of health care plans for patients.
Other duties/projects which are assigned by supervisor and/or Lead Nurse are to be followed through to completion.
External applicants Bachelor of Science in Nursing required.
Internal applicants: Associate's Degree in Nursing
Communication skills including fluency in oral and written English.
Basic computer skills including the ability to send/receive email, navigate information technology associated with the position, and use Electronic Health Record information and tools.
Current unencumbered Oregon RN license.
BLS at time of hire and must be issued by the American Heart Association (AHA) or Military Training Network branch of AHA (upload documentation with online application)
BLS must not expire during orientation or probation period, and must have the same expiration date (upload documentation with online application)
Current OHSU employees - Bachelor of Science in Nursing preferred.
Specialty Experience: 4-6 weeks.
Experience: but not in specialty area: 6 -8 weeks.
Resource: 4-6 weeks.
