The AIDS Foundation of Chicago (AFC) mobilizes communities to create equity and justice for people living with and vulnerable to HIV or chronic conditions. Our work focuses on advancing health equity, preventing new cases of HIV, serving as a collaboration and knowledge center, being a bold voice for change and strengthening our organizational excellence.
We are seeking for an individual to serve as our Resource Specialist. The Resource Specialist is responsible for assisting clients who do not qualify for case management, or who are self-sufficient in obtaining supportive services. This position will also be responsible for conducting housing screenings to link individuals with housing needs to the appropriate housing services. The Resource Specialist will work closely with the housing and care teams and act as bridge between the two programs. The Resource Specialist will be tasked with providing information to clients regarding services, addressing a variety of issues and/or providing general support, maintaining records, file, and databases. The Resource Specialist will be also be cross trained and certified in order to provide medical benefit enrollment assistance and support to clients as needed.
Principal functions and responsibilities of this position include the following:
Travel to meet clients at home, clinical, or community based setting as needed;
Identify community meeting spaces to have client meetings that are conducive to maintaining confidentiality;
Conduct face-to-face meetings with clients to assess needs;
Perform telephone screenings with clients in need of housing services;
Refer clients to case management services, and other core and supportive services if determined to be necessary;
Participate in case conferencing meeting to ensure client needs are met;
Document need for Emergency Financial Assistance and Medical Transportation;
Distribute food vouchers and transportation cards according to eligibility and need;
Complete at least 12 hours of on-going training annually;
Utilize designated client database to document encounters;
Collaborate with subcontractor agencies to schedule on-site assessments/re-assessments;
Schedule client appointments both at AFC and subcontractor agencies to complete certifications;
Travel to all funded agencies;
Perform record-keeping and clerical functions (e.g., scheduling, copying, faxing, data inputting, transmitting, telephoning, taking messages, responding to written and verbal inquires, etc.) for the purpose of supporting the needs of the client.
Assist with agency-wide activities as directed, including Annual Meeting, AIDS Run & Walk, and others;
Protect organization's value and manage risk by keeping information confidential;
Perform other duties as assigned.
Associate’s degree in a human services field OR at least one year of experience in Social Services, Health Care or related field.
Basic knowledge of HIV/AIDS prevention and treatment.
Bachelor’s degree in the Human Services field (i.e., Psychology, Sociology, Public Health, Social Work).
Knowledge, Skills, and Abilities
Strong attention to detail and the ability to provide efficient, quality service to both internal and external customers.
Knowledge of Microsoft Excel and/or other database programs. This includes the ability to generate basic reports using these programs.
Ability and willingness to respect and value the differences and perceptions of different groups/individuals.
And the ability to communicate information clearly and politely to co-workers, supervisors, and guests when speaking.

Anyone can become infected w...ith HIV, but HIV and related chronic diseases disproportionately impact certain populations in distinct geographic areas. We can have the greatest impact on the HIV epidemic by focusing our efforts on those most-impacted by HIV, based on epidemiological data and unmet need:
• Young Black gay and bisexual men
• Transgender women of color
• Black women living in high-incidence areas
• Latino gay and bisexual men
Factors that drive the HIV epidemic and health inequities that we will address include:

• Lack of access to health care
